What is Myositis?

Myositis refers to a group of inflammatory muscle diseases that cause muscle weakness, pain, and fatigue. These conditions are caused by an overactive immune system attacking healthy muscle tissue, leading to chronic inflammation, muscle damage, and weakness.

Types of Myositis (Inflammatory Myopathies)

✔ Difficulty with Everyday Movements – Standing up, walking upstairs, and lifting objects become increasingly difficult.

Necrotizing Autoimmune Myositis (NAM) – Rapidly Progressive Muscle Damage

Necrotizing autoimmune myositis (NAM) is a severe and aggressive form of myositis characterized by widespread muscle cell death (necrosis) rather than inflammation.

Severe Muscle Weakness – Rapid loss of muscle strength and difficulty walking or lifting objects.
High Levels of CK (Creatine Kinase) – A blood test shows severe muscle damage.
✔ Minimal Inflammation on Biopsy – Unlike polymyositis and dermatomyositis, muscle biopsies in NAM often show necrosis without significant immune cell infiltration.
✔ Strongly Linked to Certain Autoantibodies – Includes HMGCR antibodies (associated with statin-induced myositis) and SRP antibodies.

Necrotizing myositis requires early and aggressive treatment to prevent permanent muscle loss.

Inclusion Body Myositis (IBM) – Muscle Wasting in Older Adults

Inclusion Body Myositis (IBM) is a slowly progressive muscle disease that affects older adults, usually over the age of 50. Unlike other forms of myositis, IBM does not respond well to standard treatments.

Asymmetric Muscle Weakness – Unlike polymyositis, IBM weakness is often not symmetric.
✔ Affects Hands and Legs – Grip strength weakens, and quadriceps (thigh muscles) atrophy, leading to frequent falls and difficulty walking.
✔ Mild to No Pain – Unlike other myositis types, IBM may not cause significant muscle pain.
